Limited distribution medications

Due to patient safety concerns from manufacturers and the Food and Drug Administration (FDA), limited distribution (LD) medications are restricted to either one or a very limited number of pharmacies.

Request process for limited distribution medications

  1. The provider contacts the preferred Kaiser Permanente contracted pharmacy by telephone to order the LD medication.
  2. The preferred contracted pharmacy obtains manufacturer or FDA required patient-specific information from the provider.
  3. The preferred contracted pharmacy contacts Kaiser Permanente's Drug Benefit Help Desk to obtain prior authorization.
  4. Once the LD medication has been approved, the preferred contracted pharmacy will electronically submit a claim for adjudication and payment..

Additional information

All listed LD medications require prior authorization by Kaiser Permanente before a patient can obtain the medication.

Members currently on a LD medication from a non-preferred pharmacy are "grandfathered" and may continue to obtain their LD medications from their current pharmacy.
