Kaiser Foundation Health Plan of Washington Options, Inc. Federal PPO Plan

The Kaiser Foundation Health Plan of Washington Options, Inc. (KFHPWAO) Federal PPO provides access to an expansive network of local, statewide, and nationwide participating providers.

Provider Network

In Network

• Kaiser Permanente medical offices
• Kaiser Permanente contracted network providers
• First Choice Health in Washington, Oregon, Idaho, Alaska and Montana.
• First Health Network in all states nationwide except for Washington, Oregon, Idaho, Alaska and Montana.

Out of network

• Members can access out-of-network care from any licensed provider, medical facility, and hospital in the U.S.

Dental services

• Members on the Options Federal PPO Federal Employee plan can receive their dental care from any licensed dental provider in the U.S.

Primary Care Physician (PCP)

• Members may elect to have a PCP, but are NOT required to do so.

Referrals

• Referrals are NOT required.
• Members can SELF-REFER to medical specialists and alternative care specialists (chiropractors, acupuncturist/East Asian Medicine, or naturopaths).

Authorizations

• The member’s plan may require prior authorization for some covered services and certain medications. Please contact the Kaiser Permanente Review Services at 1-800-289-1363 or Provider Assistance at 1-888-767-4670 for more information about the plan requirements.

Prescriptions

• The Options Federal PPO plan offers formulary-based pharmacy benefits. Some plans do provide limited coverage for non-formulary medications. Please contact Kaiser Permanente Member Services at 1-888-901-4636 or see Pharmacy Services on kp.org/wa for more information.
• Members with a pharmacy benefit may fill their prescriptions at any Kaiser Permanente Pharmacy, any Optum Rx participating pharmacy or through the Kaiser Permanente Mail-Order Pharmacy.
