Kaiser Permanente offers the following educational and support services for our members.

Health Profile and support for your goals

The Health Profile is an online questionnaire and report that can help our members take better control of their health. This is a useful tool for discussing a member's overall care. The Health Profile asks about medical history and lifestyle routines. We encourage members to complete the profile yearly, to track their progress. Patients may also choose to print their results report and bring it to their visit with you. Members must register for Kaiser Permanente member website.

Case management

Our nurses serve as case managers for members whose needs are complex or when resource allocation decisions are required. Case management may cross many settings throughout an episode of illness or treatment. Case management nurses can follow a medically fragile patient after discharge to ensure smooth transition to the next level of care. They are also able to work with patients in advanced stages of chronic illness to understand how to cope with their disease and provide symptom management. Members can call toll-free, 1-866-656-4183 between 8 a.m. and 5 p.m., Monday through Friday.

By registering with the Kaiser Permanente member website, members can order prescription refills (with free shipping), check coverage and benefits, and get access to their Health Profile — all online. If members also see providers at Kaiser Permanente medical offices, they can send secure email to their care team, see lab and test results, and access their Kaiser Permanente medical record.

Health & wellness resources

Our health and wellness resources offer extensive articles on fitness, nutrition, medical conditions, mental health, diseases, and preventive care. This section includes interactive tools, videos, and the Health encyclopedia®, with in-depth patient health information, covering over 4,000 medical conditions, symptoms, and medical procedures.

Wellness coaching by phone

Wellness coaching by phone is available to all Kaiser Permanente members at no cost.

Members can receive coaching for stress management, physical activity, weight management, and healthy eating. The coaches are Kaiser Permanente staff trained in motivational counseling techniques, and provide personal guidance to help members achieve specific wellness goals.

Each member who participates in the program has unlimited sessions with a dedicated coach. Phone sessions last about 20 minutes. During the first call, the coach helps assess a member’s readiness and motivation to make change. Follow-up sessions are then scheduled when it’s convenient for the member.

Coaches can also connect members with other Kaiser Permanente resources to help them achieve their goals. Wellness coaching by phone is available in English and Spanish, no referral necessary.

Classes and events

We frequently sponsor classes, support groups, and events at our medical centers and in the community. Members can find senior fitness opportunities, such as Silver&Fit® and EnhanceFitness, as well as childbirth and parenting classes. They can join support groups or classes that focus on a medical condition or lifestyle changes, such as chronic conditions or bereavement. Some classes and events are open to the general public; check listings for details.

Patient health education materials

Kaiser Permanente produces patient education brochures, handouts, booklets, and self-management resources, such as action plans and "wallet card" tracking tools. This site includes handouts in PDF format for easy printing. Topics include children's and teen's health and safety, immunization and preventive care schedules, men's and women's health, and pregnancy.

You can request our wallet cards and booklets from Provider Services. These materials include record-keeping tools to record blood pressure, carbohydrate consumption, immunizations, and more. Booklets address tobacco cessation, teen topics, managing alcohol use, and more.

24/7 advice

When members want care advice or need to know if they should get immediate medical attention, our 24/7 advice line can help. A nurse will listen to the member's concerns, and if necessary, direct them to the best location to get care.

Community resources

KPWA assists member in connecting with community resources to support social health needs such as housing, food, and financial assistance. Members can review educational materials, search a directory for resources near them, or contact a Kaiser Permanente resource specialists at 1-800-260-7445 (TTY 711) Monday through Friday from 8 a.m. to 5 p.m.
