Medicare fracture outreach
Kaiser Permanente assists with identifying and offering treatment opportunities for members with potential osteoporosis-related fractures.
Primary care providers are notified monthly of any new fractures reported for Medicare patients who have not yet had a bone density scan to evaluate risk or medication to treat osteoporosis.
Medicare patients with new fractures are mailed a letter encouraging them to talk to their doctor about getting a bone density scan or medication to treat osteoporosis.
