Care gap reminders

Reminders are sent to members about a number of care gaps, including

• Cancer screening (breast, cervical, colorectal)
• Diabetes maintenance (A1c, eye health, kidney health)
• Osteoporosis care after a fracture
• Child well visits and vaccinations

Reminders may be in the form of letters or online secure messages for those who have access.
