Quality performance reports

Monthly quality performance data is available for all contracted primary care providers. The reports include performance data for many HEDIS^® clinical quality measures. Providers use these lists to compare against their own data and to evaluate their organizational outcomes and priorities.

Care gap report

The Care Gap Report provides monthly patient-level information for HEDIS^® clinical quality measures. These patients are past due for certain care, tests, or medications.

Providers use these lists to support patient outreach, to coordinate addressing care gaps and to compare against their own data.
