Requesting preauthorization for coverage

*Note – this section does not apply to Mental health, including addiction and recovery

Using the referral request application

Kaiser Permanente Washington's preferred method for requesting authorization is through the Referral Request tool Padlock on our provider web site. You can access and use the Referral Request tool by logging in through One Health Port. If you don't have an account, visit OneHealthPort. Requests received via this application may be auto adjudicated, giving you an instant response to your request.

If you are not able to log in to our provider website through One Health Port, we offer some additional methods to request authorization.

We have two methods for submitting referrals online. Please use the hyperlinks below to access a job aid with instructions regarding use of each method.

  1. Referral/order entry (used by most contracted providers)
  2. Manual/exception referral (used by non-contracted providers and a designated subset of contracted providers)

Calling Review Services

Staff in Kaiser Permanente’s Review Services department are available to accept your requests for authorization at 1-800-289-1363. We are available Monday through Friday from 8 a.m. to 5 p.m. PST.

*Note – questions regarding what services require authorization or authorization status should be directed to our Provider Assistance Unit at 1-888-767-4670.

Kaiser Permanente Washington does not accept faxes as initial requests for authorization from contracted providers. Contracted providers should always use the Referral Request application or call Review Services directly.

A provider who is not contracted with Kaiser Permanente Washington can contact Review Services during business hours at 1-800-289-1363.

If a non-contracted provider doesn't have access to the Referral Request tool but they need to request authorization outside of normal business hours, use this fax form (PDF). We will review your request during normal business hours.

Checking the status of your request for authorization

Any provider with a One Health Port login can use the Referral Status Inquiry Padlock application to check the status of an authorization 24 hours a day, seven days a week.

You can also contact our Provider Assistance Unit Monday through Friday from 8 a.m. to 5 p.m. PST at 1-888-767-4670 to check the status of an authorization request.
