Kaiser Permanente Commercial and Medicare Advantage HMO networks

Kaiser Foundation Health Plan of Washington and Kaiser Foundation Health Plan of Washington Options, Inc. (Kaiser Permanente) offers services in Washington and Idaho. Washington counties include Benton, Columbia, Franklin, Island, King, Kitsap, Lewis, Mason, Pierce, Skagit, Snohomish, Spokane, Thurston, Walla Walla, Whatcom, Whitman, and Yakima. Idaho counties include Latah and Kootenai.

We are not accepting unsolicited applications for new contracts in the Washington service area at this time. We will only reply to requests that meet a need within our networks.

We are accepting applications for new contracts in the Idaho service area (Latah or Kootenai counties only). If you provide services in Latah or Kootenai county, please submit a Provider Information Form

To apply for a new contract in a region outside of Washington, please refer to the Kaiser Permanente Regional Provider Relations Key Contacts page on the national Kaiser Permanente website for the appropriate regional contracting department.

Kaiser Permanente PPO networks

Currently, we have very limited needs within our PPO networks. Applications are only considered for specific network needs in our service area. We do not offer the PPO plan in Idaho. If you are interested in joining our PPO network, submit a Provider Information Form. We only respond to unsolicited applications if your request meets a current network need.

Mental Health & Wellness providers

We are currently accepting applications for new contracts in select counties only within Washington, and in Latah and Kootenai counties in Idaho. Please review the network status matrix (PDF) to see which counties are open to accepting new providers. If you meet the criteria listed in the matrix, please submit a Provider Information Form (PIF) online.

We are not accepting unsolicited applications for providers located outside of our Washington and Idaho service areas. This is the case even if you’re offering telehealth services.

Submitted PIFs will be reviewed based on Kaiser Permanente’s business and clinical needs. Submission of a PIF does not guarantee that your application will be approved. You will be notified about whether your application is approved or denied. If your PIF is approved, the initial credentialing and contracting process will begin at that time.

You may not see Kaiser Permanente members until 1) You have completed and been approved by credentialing and, 2) Signed and returned a contract (if a new contract).

Alternative Medicine and Chiropractic providers

WholeHealth Networks is the administrator of Kaiser Permanente’s alternative medicine network. All requests for additions to current contracts should be submitted to WholeHealth Networks, Inc. (WHN), a subsidiary of Tivity Health Support, LLC, at 1-800-274-7526 or email contactus@tivityhealth.com.

Delegated provider groups

If your organization is under a delegated credentialing agreement (see Delegated provider list), notification of changes, adds, terms, and updates must be sent via the standardized delegated roster notification process.
