In order to be in-network for commercial PPO members you must have completed and passed Kaiser Permanente's credentialing process and have a Kaiser Permanente contract.

Much of the information on this site can be accessed without signing on.

For information regarding member eligibility, referral requests, and claims status, sign on through OneHealthPort. Please note all links may not apply to all plans.

Identifying Members

We issue ID cards to all members. This card lists the member's name, ID number, and health plan network.

To check member eligibility and benefits sign on using your OneHealthPort credentials and click Eligibility Inquiry . An Eligibility Inquiry demo is available to assist you.

Review Services & Clinical Review

While referrals are not required for this product, prior authorization is required for some services. View a list of services that require prior authorization:

Kaiser Permanente accepts prior authorization requests using the Referral Request tool. Please note a OneHealthPort login is required to access this.

Pharmacy

See our Kaiser Permanente PPO drug formulary. Use the Exception Request Form for medications that require prior authorization. PPO members can use our Mail-Order Services and benefit from significant savings.
